What Is a Vertebral Endplate?

Vertebral Endplate is a thin layer of cartilage and bone between a spinal disc and the nearby vertebral body. Each endplate helps connect the disc to the spine and supports movement. Endplate changes may appear on imaging and can relate to low back pain when nearby nerves become involved.

Key Takeaways About Vertebral Endplate

  • It sits between a spinal disc and a vertebral body.
  • Endplate changes may help explain certain types of low back pain.
  • An endplate is not the same structure as a spinal disc.

How Vertebral Endplate Works

Vertebral Endplate

A vertebral endplate forms the boundary between a spinal disc and a vertebral body. The cartilage layer helps spread pressure across the disc. The bone layer helps anchor the disc to the vertebra.

Endplates also support the disc's exchange of nutrients and waste products. Small nerves can reach the endplate area through nearby bone. When an endplate changes, imaging may show the change near a painful spinal segment.

Basivertebral nerves carry signals from the vertebral body and nearby endplate region. Basivertebral nerve ablation targets these nerves when a clinical evaluation links them to chronic vertebral pain. Imaging findings alone do not identify the pain source.

Vertebral endplate

From the Premier Injury Specialists team

When we assess vertebral endplates, we review imaging alongside the pain pattern and examination findings. Our team checks whether the endplate region matches the suspected pain source before discussing Basivertebral Nerve Ablation. We also separate endplate findings from disc, facet joint, and nerve-root problems.

Vertebral Endplate in Practice: A Real-World Example

A person with ongoing low back pain may have endplate changes near a painful vertebral segment on imaging. During evaluation for Basivertebral Nerve Ablation, Premier Injury Specialists compares that finding with the person's symptoms and examination.
